Could you help me understand something that happened early this morning around 5 a.m.? I was asleep, and suddenly I had some sexual thoughts. Right then, sperms were ejected from my penis. Is this something normal, or could there be an issue that I need to be worried about?

It's completely normal for sexual thoughts or dreams to lead to spontaneous ejaculation, especially during sleep. This is a natural physiological response, often referred to as a "nocturnal emission" or "wet dream." It doesnt indicate any underlying health issue and is a common occurrence for many people.

Normal physiological process:
It is quite common and normal for young males around the age of 19 to experience spontaneous ejaculation during sleep, often called "nocturnal emission" or "wet dreams." This usually happens after sexual thoughts or stimulation and is a natural part of sexual development. -
No cause for concern:
Nocturnal emissions are a way the body releases built-up semen and are common during adolescence and early adulthood. Unless accompanied by pain, discomfort, or other unusual symptoms, this is not a medical issue. -
When to seek medical advice:
If there are additional symptoms like pain, frequent involuntary ejaculations causing distress, or other urinary or reproductive problems, consulting a healthcare professional is advisable.
Overall, spontaneous ejaculation during sleep after sexual thoughts is a normal experience for many young men and does not typically indicate any health problem.

I have left reanal hydronephrosis with a 7mm ureteric calculus in the lower part and a small 3mm concretion in the lower calyx of my left kidney should I consider medicine or is surgery the better option here
Given the size of the ureteric calculus (7mm) and the presence of hydronephrosis, surgery is likely the better option for your situation. While a 3mm concretion in the lower calyx may be managed medically, the 7mm stone in the ureter and the associated hydronephrosis suggest a need for more definitive treatment.
