Hello doctor. This is my ultrasound report of the neck. Please check if it is normal. What is the treatment for this. Do I need any treatment for this. Is this malignant?

My wife is currently taking Thyronorm 75 mcg. Before conceiving, her TSH level was 3. Now, at 5 weeks pregnant, her latest TSH level is 4.73. The reference range for TSH in the first trimester is below 4.5. Could you please advise if this level is a concern and if any changes to her medication or treatment are needed?

I have been taking thyroid medication since the age of 11 and have never missed a dose. I am now 23, and my menstrual cycles have always been regular. My TSH is currently below 1, and it has been maintained below 5 for several years. Will this affect my ability to get pregnant? Also, does this increase the risk of miscarriage?
