i had open heart surgery back in 2009 and also have a history of uti e coli but currently not on any meds since the doctor stopped them recently but for the past two days ive been having really bad pain on the left side of my lower back with some slight swelling could this be something serious

I've been dealing with cuts on my foreskin for about a week now, and they're just not healing no matter what I try. I'm feeling a burning sensation when I pee and it's been happening for around 34 days now. There's also this white discharge coming from my urethra. Any idea what might be going on?
