I have a grade 1 posterior medial meniscus tear will this heal on its own or do I need surgery? I'm worried about making it worse if I don't get the right treatment. What's the usual recovery time for something like this?

Can someone help me understand if surgery is really needed based on this MRI report? The person is currently on meds but the doctor hasn't recommended physiotherapy yet just exercises. The neck pain started in April and keeps coming back, even shooting down to the right arm. Why would the doctor only suggest exercises and not physio?
