I have grade 4 varicocele and azoospermia with zero sperm count could the varicocele be causing this? IVF clinics are recommending TESA since I'm trying for a child, but I'm unsure whether to go for varicocele surgery first or proceed directly with TESA to improve my chances of having sperm

I've been feeling a bit off lately like there's this strange discomfort or numbness in my right testicle. My left hydrosol is three times bigger than the right one and has been painful for the past two days. It also feels harder compared to the right side. Should I consider surgery or are there any medications that can help with this?
Painful hydrosol may respond to anti-inflammatory meds temporarily—if large or recurrent, surgery is the definitive solution.
