I can't pull back my foreskin when I'm erect it hurts if I try to force it, but when it starts going back to flaccid, I can retract it easily since it doesn't feel tight then. Is there any way to fix this without needing surgery?

In severe cases of paraphimosis, circumcision may be recommended. In very severe cases, a lack of blood flow to the penis can cause tissue death (gangrene) and you may need surgery to remove the penis.
