I've had my hair smoothened twice, and now I'm noticing bald patches and a lot of hair fall. I also have PCOD, so I'm really worried if that could be making things worse. Could the treatments have caused this, or is it more related to my PCOD? What can I do to improve my hair health?

In PCOD your hair will be week hence do not try such methods which would cause more hair loss,visit dermatologist for appropriate managemnt.
