I've had a naturally thick lower lip since childhood but now at 19 it seems to be getting even thicker while my upper lip stays small. Are there any natural ways to reduce the size of my lower lip without surgery

this looks like cheilitis which is due to vitamin deficiency , consume balanced diet and more green leafy vegetables,and also stay hydrated, never scratch or try to peel it off.
