I've had varicocele for about a year now but lately the pain and swelling have gotten really bad plus my sperm count is low The urologist recommended surgery but I'm not sure what to do or if there are other options

I've been experiencing some tender pain at the top of my left testicle, and sometimes it even seems like the shape of it isn't quite round. Walking or touching that area makes the pain worse.
