I've had a voice disorder since birth due to a cleft palate, and my speech isn't clear nobody understands me. I've tried speech therapy multiple times but nothing changed. Could there be an underlying vocal issue surgery might fix? I don't want more therapy if it hasn't helped before. What options do I have to improve my voice clarity?

I'm noticing a yellowish color on the roof of my mouth, and it's been bothering me. I've also been having a persistent cough and there's this hard area up there that has me a bit concerned. I've had a sore throat for the last three months and it's not getting better. I've tried different medications like Bactoclav 625mg and Cefixime, but nothing seems to work. Should I be worried or try something else? What do you recommend?
