I've not been able to have a bowel movement for the last four days, even though I've been taking Cremaffin syrup, and it doesn't seem to help. My stomach feels really heavy and painful. What should I do?

I'm feeling some really sharp pain under my ribs, like being poked with a nail. It's happening between my 3rd and 2nd last rib, and I've been dealing with it for about 45 years. Sometimes it happens every 2-3 months, but other times it's more regular. I'm starting to worry if it might be something wrong with my liver, or maybe it's just gas-related? I also have constipation issues. What could be going on?

I'm really worried about my wife. She's been dealing with this pain on the right side of her chest for a couple of weeks now. We had an ultrasound and X-ray done, and the doctor mentioned something about liver enlargement and fluid in her lower lungs. The reports also showed WBC lymphocytes at 20 and neutrophils at 76, plus there's an infection in her urine. We've tried different medications, but her pain hasn't gotten much better. What should we do for treatment? Any advice would be really appreciated.
