My father-in-law is 55 and a farmer, pretty fit overall. He's had swelling and pain below the scrotum for about a year, and now the pain is getting worse. A doctor said it might be a hernia. At what point does he need surgery? Are there any meds that can help avoid surgery? Is it better to get surgery done early?

I'm also worried about how this might affect my sexual life since I'm not married yet. What could happen if I don't get it treated now will it cause more problems later?
Craniotomy means opening the skull to remove a clot or relieve pressure; recovery depends on damage extent but early surgery improves outcomes.
