My L45 disc shows a right paracentral focal disc extrusion that's 14 x 10 x 12 mm, pressing on the thecal sac and cauda equina roots. There's mild central spinal canal stenosis, moderate right and mild left lateral recess stenosis, compressing the right L5 root and touching the left L5 root. The L5S1 disc has a posterior central protrusion too. Do I really need surgery or are there other treatments that could help?

View allMy MRI shows disc desiccation and a large posterocentral disc protrusion at L4-L5 compressing nerve roots and effacing the thecal sac with mild facetal arthropathy. Does this require surgery or can it be managed with medication? Also noticed focal hemangiomas in D10 and D12 vertebrae should I be concerned about those? The rest seems normal but I'm worried about the compression
Disc protrusion with nerve compression may need surgery if symptoms are severe—hemangiomas are usually harmless unless symptomatic.

I've had a slipped disc at L5 S1 for the past year and recently developed sciatic nerve compression on my left side. The EMG NV test confirmed nerve compression, and now I have swelling, numbness, and tingling in my left foot. My doctor recommended surgery, but I'm currently taking multivitamins, calcium, and methylprednisolone. Are there any alternative therapies that could help with recovery?
