What exactly happens during keyhole surgery for a slipped disc do they remove the whole disc or just part of it I'm a bit confused about how much they take out

It's a mixed-intensity lesion with cystic and hemorrhagic areas, about 5.3 cm in size, showing low T1 and high T2 signals on MRI. My father had subdural hematoma surgery two months ago and the doctor said CSF is collecting in his brain, so they did a lumbar puncture twice in two days. Is lumbar puncture usually used to treat CSF buildup? Now I'm noticing a deep dent in his skull should I be concerned about that?
