Question
I had gallbladder surgery one and a half years ago. Since then, I have been experiencing frequent indigestion, malabsorption, itching, weight loss, foul smelling stools, and occasional diarrhea. A CT abdomen done a year ago suggested possible IBS. Recently, I had an MRCP, which showed a bulky pancreatic head but no focal spots or inflammatory lesions. My amylase levels are consistently above the normal high range (around 50+). I also did a lipid profile which shows triglycerides are 320 (high), total cholesterol, VLDL, and LDL are slightly elevated. In my urine test, protein and sugar traces were found, but my RBS is normal. CA 19 to 9 is 4.20 U/ml. Is there any possibility of pancreatic cancer now or in the future? My diet is balanced, I avoid fried and oily foods and consume minimal fat and protein. I am feeling very anxious about this. Please help.

Your CA19 to 9 is normal (4.20), which is reassuring and does not suggest pancreatic cancer currently. A bulky pancreatic head without masses on MRCP also does not indicate cancer. High amylase, indigestion, foul smelling stools, and weight loss after gallbladder surgery may suggest chronic pancreatitis or exocrine pancreatic insufficiency rather than cancer. Triglycerides are high, which can impact pancreatic health. Please follow a low fat diet, avoid alcohol, and continue hydration. You can book Serum Lipase, Fasting Blood Sugar, HbA1c, and a follow up with your gastroenterologist for pancreatic enzyme assessment.
