What does big blood clots in period mean?

Blood clots, when they are small and only occasional, is common and normal on heavy flow volume days. Found a pea-sized lump in my right breast and the mammogram says fibroadenoma BIRADS 2. Do I really need surgery or are there medicines that can help? I'm really concerned about whether this could turn into cancer later

BIRADS 2 means benign fibroadenoma; usually doesn’t need surgery unless painful or enlarging.
