I've been getting these red dots on the head of my penis after having sex or masturbating and it's starting to hurt when I try to pull back the foreskin, especially around the frenulum. The weird thing is, if I don't do anything for about 45 days, the dots go away. I went to a dermatologist who said it might be a fungal issue and gave me an antifungal cream, but it's been a month and nothing's changed. I've tested negative for HIV. Any idea what this could be and how I might treat it?

I recommend trying a combination of antibiotic and steroid cream to address any possible bacterial or inflammatory component of the condition.
