This is the fourth time this year I am experiencing frequent urination. Previously, I visited a physician who prescribed antibiotics, and the issue went away for a while, but it keeps recurring. I have attached my urine report. Please suggest what I should do.

Recurrent UTI is possible. Consider urine culture and ultrasound to check for underlying causes (e.g stone, prostate issue). Urologist review is recommended.
