I have a painful swelling near my anus but no bleeding, is surgery the only option or can it be treated another way? Also what foods should I stay away from to help with this?

I'm really worried about my piles. It's been 23 years since they first showed up, and lately, I've been having a lot of pain and irritation during and after going to the bathroom.
