I'm constantly feeling the urge to pee and poop. It's really frustrating because when I try to pee, I have to push really hard and only a tiny bit comes out. When I poop, its incredibly painful because my anus is really swollen and inflamed. I'm stuck in the bathroom and it's becoming unbearable. What's going on with me? Can you help?

Possible Diagnoses*

  1. Urinary Tract Infection (UTI): Frequent urination, straining while urinating, and minimal urine discharge are common UTI symptoms.
  2. Prostatitis: Inflammation of the prostate gland can cause urinary symptoms, including frequent urination and straining.
  3. Proctitis: Inflammation of the rectum can cause painful bowel movements, swelling, and inflammation around the anus.
  4. Anal Fissure: A small tear in the lining of the anus can cause painful bowel movements, bleeding, and swelling.

Treatment Advice

  1. Seek Medical Attention: Consult a doctor or a gastroenterologist to determine the underlying cause of your symptoms.
  2. Urinalysis and Stool Tests: Your doctor may order these tests to check for infections, inflammation, or other conditions.
  3. Antibiotics: If a UTI or prostatitis is diagnosed, your doctor may prescribe antibiotics to treat the infection.
  4. Pain Management: Your doctor may recommend pain relief medications, such as acetaminophen or ibuprofen, to manage discomfort during bowel movements.
  5. Stool Softeners: To reduce straining during bowel movements, your doctor may recommend stool softeners, such as docusate or psyllium.
  6. Warm Sitz Baths: Soaking in a warm bath can help reduce inflammation and discomfort around the anus.
  7. Dietary Changes: Increasing fiber intake, staying hydrated, and avoiding spicy or irritating foods can help manage symptoms.

When to Seek Emergency Medical Attention

  1. Severe Abdominal Pain: If you experience sudden, severe abdominal pain, seek immediate medical attention.
  2. Vomiting Blood or Black Tarry Stools: If you vomit blood or pass black, tarry stools, seek emergency medical attention.
  3. Fever Above 101.5F (38.6C): If you have a fever above 101.5F (38.6C), seek medical attention.

How do I properly massage my penis and testicles, and is it safe to use coconut oil for this? I've been curious about whether there's a right technique and if coconut oil is a good option.

To massage your penis and testicles, use gentle, circular motions without applying excessive pressure. Coconut oil can be used as a lubricant due to its moisturizing properties, but ensure you're not allergic. If you have any concerns or discomfort, its best to consult a healthcare professional.

What's the best surgery option for a grade 3 varicocele and how is open surgery done do they use any magnification during the procedure

For grade 3 varicocele, microsurgical varicocelectomy is generally considered the best surgical option, offering the highest success rate and lowest risk of complications. Open surgery, specifically microsurgical subinguinal varicocelectomy, involves making a small incision in the groin area, and the surgeon uses a microscope or loupes (magnifying glasses) to identify and ligate (tie off) the enlarged veins, preserving the testicular artery and lymphatic vessels.
