I'm noticing this really dark black layer that keeps forming on just my nose and chin. It's only happening in those areas and it's starting to worry me. What could be causing this, and should I be concerned?

I've been having this really annoying itching problem all over my body that only seems to happen at night. I saw a doctor about it and took the medication they prescribed, but the itching came back.
