I'm really concerned because I'm experiencing severe early ejaculation. I can get an erection just fine, but I can't seem to maintain it long enough as I end up ejaculating just 15 to 20 seconds after the erection. What could be causing this and what steps can I take to address it?

I've had tight foreskin since I was a kid and thought it would loosen over time if I kept pulling it back, but now I'm 25 and it's still really tight. There's only about a 34mm opening to pee through, and I get this white stuff building up that causes itching. I'm worried surgery might remove the whole foreskin are there other options that can fix this without taking it all off?
