I've been diabetic and recently noticed my left testis is enlarged but there's no pain. The ultrasound showed moderate fluid collection in the left scrotum. Can this be treated with medication or will I need surgery?

I have an 11mm kidney stone in my upper pole and need laser surgery to remove it. I really don't want to be put to sleep for the procedure can I just get local anesthesia instead?
