My grandmother recent blood reports show the following, sodium level is 119, CRP is 238, and postprandial blood sugar is 582. She also has hypothyroidism and is currently on medication for ischemic heart disease, cholesterol, and hypertension. She is extremely weak and has lost her appetite. Are these report values very serious, and what should be done next?

Very low sodium, high CRP and sugar indicate critical condition. Needs hospital care, electrolyte correction, infection control. Monitor closely.
