It is 30 point questionnaire to assess the cognition level of a person. It includes assessment of orientation to time (5 points), orientation to place (5 points), registration (3 points), attention and calculation (5 points), recall (3 points), language (2 points), repetition (1 point) and complex commands (6 points). Any score of 24 or above indicates normal cognition. Below this, scores can indicate severe (â¤9 points), moderate (10â18 points) or mild (19â23 points) cognitive impairment.

I have this weird sensation of water in my head and constant tightness, along with sensory OCD. I've just gotten my MRI results and I'm feeling anxious about what it all means. It says there's a diffuse disc bulge at L4-L5 causing thecal sac indentation and compression of both existing nerve roots. Also, at L5-S1, it mentions a similar disc bulge with indentation and nerve compression including bilateral neural foramina. I'm worried about what kind of symptoms or issues this could lead to.
