Which is the best test to detect appendix inflammation? If a CT scan is performed, is it better to do it with contrast or without contrast?

The best test to detect appendicitis is a CT scan with contrast, as it clearly shows the inflamed appendix, surrounding fat changes, or complications like abscess. Its more accurate than CT without contrast in adults. However, in children, pregnant women, or if you have kidney problems or dye allergy, an ultrasound or non contrast CT may be used. For most adults, CT with contrast is the preferred and most reliable choice.
