Dear Doctor, I am a 40 year old working woman with a history of rheumatoid arthritis (on medication for 10 years) and hypothyroidism. Over time, I have noticed a gradual loss of interest in sex. After a day work, I usually feel very tired and go straight to bed. Even on weekends, intimacy is rare. I also experience vaginal dryness most of the time, which makes penetration painful. I rarely get aroused, and when I do, it is usually in the mornings, but that is not an ideal time due to privacy concerns with our child. My questions are: Is there any way I can improve my energy levels in the evenings so that we can enjoy intimacy? How can I address vaginal dryness and make sex more comfortable? Are there ways to improve my arousal, performance, and satisfaction despite my health conditions (I can only manage missionary position due to joint issues)?
