This test is used to measure the cortisol levels in blood, which is released by the adrenal glands located over the kidneys. Cortisol helps in regulating the metabolism of carbohydrates, fats, proteins and also in the regulation of glucose levels in the blood. The levels of cortisol may change during a stressful response. The levels are usually high in the morning and gradually fall by evening/night time. The test is used to detect if the adrenal gland is hyperactive or underactive. This sample is mostly and preferably collected early in the morning. High levels of cortisol may cause Cushing's syndrome and low levels may lead to Addison’s disease.

How is the blood sample taken?

To take the blood sample, a tourniquet (elastic) band is placed tightly on the upper arm. The patient is then asked to make a fist. This helps in the build-up of blood filling the veins and it becomes easy to collect the blood. The skin is cleaned before inserting the needle in the vein in order to prevent bacteria from entering. Needle is inserted into the vein of the arm and the blood sample is collected in the vacutainer.

Cushing?s syndrome or hypercortisolism, occurs due to abnormally high levels of the cortisol. The symptoms include: weight gain, fatty deposits, Moon face (round and full face), Buffalo hump (fat bulge between the shoulders), Gaining weight around your belly, Skin becomes thin, High blood pressure, High blood sugar, acne, fatigue, muscle weakness.

What is Addison?s disease?

Addison's disease, also called adrenal insufficiency, with low level of cortisol. Symptoms include: Fatigue, weight loss and decreased appetite, hyperpigmentation, low blood pressure, low blood sugar (hypoglycaemia), nausea, diarrhoea or vomiting, abdominal pain, muscle or joint pains, depression or other behavioural symptoms, body hair loss or sexual dysfunction in women.

A tumour in the brain?s pituitary gland can trigger your body to make too much cortisol.
