The white blood cells (WBCs), also known as leukocytes, are essential to the immune system. The cell’s ability to fight against infections depends on the WBC count in the blood. In an adult person, the ideal WBC count should be between 4500 and 11000 per microlitre of blood.

Someone with a high WBC count would have leukocytes of more than 11000 in a microlitre of blood. This condition is known as leukocytosis. It can be of the following types:

  • Lymphocytic leukocytosis: Lymphocytes are a type of WBC that are a vital part of the immune system. These leukocytes ward off parasites, fungal growth, viruses, and bacteria. However, a high count of these cells is the most common cause of two known types of blood cancers: lymphocytic leukaemia and lymphoma. 
  • Neutrophilic leukocytosis: Neutrophils are another type of leukocytes or WBCs. These also provide protection against fungal growth, bacteria, and parasites. Neutrophils also play a vital role in the injury healing process. However, the number of neutrophils can increase due to myeloid leukaemia, causing an increased number of immature neutrophils in the bloodstream. 
  • Non-malignant leukocytosis: WBC disorders can be non-malignant as well as malignant. The non-malignant type of leukocytosis is mostly infections in the body. These can occur due to poor lifestyle choices like smoking, obesity and genetic conditions such as Down’s syndrome. 
  • Malignant leukocytosis: Malignant leukocytosis is commonly referred to as blood cancer and causes sweating, rapid weight loss, and fever. Other symptoms include swollen liver, lymph nodes, and enlarged spleen.

Total leukocyte count (TLC) testing is done on the blood sample of the patient. The blood is collected from veins and stored in a vial tube. Then the sample is sent to the lab for testing. This test tells the exact amounts of WBCs in the blood. There is no need for fasting or other requirements for this test.
The blood sample collection process for a DLC test is similar to the TLC test. Once the sample is sent to the lab, the specialist puts a drop of blood on a clear glass side and smears it. By spreading the blood droplet around, they can dye it and distinguish the WBCs. You do not need to fast for this test.
You may or may not need to fast for an ESR test. The sample collection process for this test is the same as a TLC/DLC test. However, in this test, the erythrocytes or RBCs are tested. It is mainly done to check the progress of inflammatory diseases in the body.
A higher than average erythrocyte sedimentation rate indicates possible inflammation within the body. This occurs as a response of the immune system to the inflammation caused by an infection or an injury. The inflammation can also result from an underlying chronic disease or immune disorder.
A high TLC result indicates an elevated count of WBCs in the blood. This can occur when an infection has entered the body, and the immune system is fighting it. Other times, this can be an indication of emotional or physical stress. Not all cases of high WBC are malignant. However, it has been seen that patients with blood cancer tend to have an elevated WBC count.
The total leukocyte count (TLC) can be affected by different disorders. For instance, a congenital disorder that destroys bone marrow function can cause a lower WBC count. Autoimmune diseases or other severe infections too can use up the WBCs before they are produced, reducing their count.
