Found a 76 mm HU 984 stone in my upper ureter causing mild hydroureteronephrosis and some fat stranding near the pelvic ureteric junction. Wondering if this can be treated with meds or if surgery is needed?

I have mild varicocele but not much pain do I really need surgery or can I manage without it
