I had a deep wound about 4 inches long and 3 inches deep after my pilonidal sinus removal. It's been 20 days and the wound is still open my doctor suggested flap surgery but I'm hesitant to go under the knife again. If the wound heals on its own over time, could there be any long-term issues compared to getting stitches or surgery? I'm worried about complications later in life if it heals naturally

The surgery wound from my perianal pus removal a month ago hasn't fully healed yet and there's a small bump on one side - could that be fat? I attached a picture, how much longer might it take to heal completely and what can I do to help it heal faster so I can get back to normal
Bump may be granulation or fat—keep clean, sitz baths, and recheck with surgeon if healing doesn’t progress.
