I had fistula surgery 6 years ago but the problem is back again for the past year with pain, swelling, and pus or blood discharge. I'm thinking of getting laser surgery this time to fix it for good can you recommend a good clinic or doctor for this?

I've been having a lot of swelling and really bad pain in my perineum and one side of my anus. I can feel this firm lump near the anus opening and the whole perineum area is super swollen and firm too. It's incredibly painful and sometimes I get the chills and a fever from all the pain. I've been taking paracetamol to help a bit, but the pain just keeps coming back when it wears off. It's been like this for three days now, and it's just getting worse. I tried sitting in hot water and it gives some temporary relief, but I'm worried. What could this be and how do I treat it?
