I have a 7.1x6.9mm epididymal cyst and noticed pus in my semen. I did a semen culture test and took meds but nothing changed. Are there other treatment options or would surgery be needed?

I'm really wondering about what to do next. I was using Triamcinolone Acetonide Ointment USP 0.5 for phimosis for the last couple of months while I was in America. Now that I'm in India, I'm finding it hard to find the exact same thing. I've heard about Betnovate and Diprovate G as possible alternatives. Could one of these be a good replacement, or is there something else I should consider? I'm a 29-year-old male and I've never been able to pull my foreskin back when erect, though I can stretch it to the level shown in the picture when flaccid. My main concerns are do I need surgery or can this be managed with stretching exercises and creams? Also, is this because of a tight foreskin or a short frenulum breve like in the picture?
Get it operated or start proper medication.. To get rid of this problem permanently without any side effects
