I've been dealing with a really painful and burning throat all day. Hot water and salt didn't help at all. It's so uncomfortable when I swallow, and the pain's more on the left side. Any ideas on what's going on or how to make it stop?

I've been dealing with a swollen left lingual tonsil for about a month now. Initially, it wasn't painful, but now I'm feeling some discomfort when I swallow and I have ear pain too. There are also these white spots on it. I'm a pretty heavy smoker. What could this mean and what should I do?
