I've had an anal fistula for 13 years and tried both allopathy and homeopathy treatments. It gets better sometimes but keeps coming back. Should I consider surgery or is there any other medicine that might work better for this?

I'm really worried because my husband and I tried anal sex recently, and it was super painful the first time, so I had to stop. Now, the next day, I've noticed a fleshy lump around the wrinkles of my anus, and it's really painful. It's messing with my normal activities. What should I do about it?
