My 2 years daughter is having fever from 5 days. We are consulting to regular paediatric doctor. Attached blood report taken yesterday. Please suggest?

He continues to take both pneumonia and diabetes medications. Sleeping in a cooler worsens his cough. He had slight relief after stopping diabetes medications for a day, but it was minimal. The doctor recommended a sputum test, but he is unable to produce sputum. What should be the next steps?
