My DPTA scan showed my right kidney is mildly enlarged and hydronephrotic but has good cortical function and passive pelvic stasis without significant obstruction. Do I need surgery for this or can it be managed without an operation?

My penis head bends downward when I pull back the foreskin during an erection, and I think it's due to a tight frenulum. It makes masturbation uncomfortable because of the tension in that area. Is frenuloplasty the only solution, or are there non-surgical options that could help?
