My foreskin feels really tight when I get an erection and it's causing discomfort. I don't have issues when I'm not erect, but during arousal, it gets too tight. Is there any way to fix this without surgery? I'd prefer non-surgical options if possible.

I had a BPH surgery in 2006 that went wrong and left me with uncontrollable incontinence after my sphincter was accidentally cut. I have bilateral grade 1 varicocele and my left testicle hangs much lower and feels heavier than the right one. The urologist prescribed hetrazan and health okay tablets, but they aren't helping with the pain and discomfort. Should I consider surgery? What's the best type with the least complications?
