My last tooth has a cavity and isn't straight it's lying sideways but hurts a lot. Do I need to get a scan and surgery for this?

I'm pretty sure I've got a canker sore on my palatoglossal arch, and it's been growing. There's no headache or stomach pain, just pain right where the ulcer seems to be. If it's not a canker sore, what else could it be? It's been around for a week now and isn't showing any signs of shrinking or healing. What should I do to treat it? I'm starting to get a bit worried.
