In my semen analysis, my sperm count was nil. I have done a scrotum scan, which was normal. I have also done tests for LH, FSH, and testosterone. What is this issue called, and is it curable?

Nil sperm count in semen analysis is called azoospermia. Since your scrotum scan is normal, hormone tests (LH, FSH, Testosterone) will help check if it is due to hormonal imbalance or a blockage. Hi Doctor, I took Saw Palmetto and Beta Sitosterol 500 mg to reduce my DHT. Since then, I have been experiencing sexual problems. I consulted a urologist who found my testicles are slightly small in size and volume by USG and Doppler. My testosterone level is 3.38 ng/ml. SHBG and diabetes levels are normal. I do not have anxiety. I can get an erection during masturbation, but the problem is that my testicles are small, I have no libido, and I do not ejaculate properly, it is just one drop. I have been facing this problem since I took Saw Palmetto. I stopped taking it, but the side effects are still the same. Should I go for hCG therapy or TRT? Whom should I consult, a urologist or an endocrinologist?

Doctor, please check my semen analysis result below and tell me what I should do? I get good erections, but I ejaculate within one minute. Time of liquefaction is 30 minutes. Colour is greyish white. Consistency is thick. pH is 8.0. Volume is 2.5 ml. Total sperm count is 84 million/ml. Morphology is 55 percent normal, 15 percent head defect, 15 percent midpiece defect, and 15 percent tail defect. Motility is 15 percent active, 30 percent sluggish, 55 percent non motile. WBC is 8 to 10 per HPF.
