View allmy husband keeps getting a perianal abscess every 23 days that bursts on its own and keeps coming back for the past 2 months. he doesn't have much pain but the doctor said it's a chronic fissure. he's really scared of surgery and the doctor hasn't suggested it yet what could be causing this to keep happening and are there other treatment options besides surgery

I've had an anal fissure with a sentinel tag since December 2018. It improves with treatment but flares up again as soon as I stop meds. Doctors say surgery is the only fix, but I really want to avoid that. Are there any other options that could help long-term without needing an operation?

Is surgery the only option for treating a fistula and is it actually safe and effective Also can the muscles that control stool be preserved if it's a complex fistula I'm really worried and need some advice
