My wife sometimes bleeds when passing stool and has skin tears on three sides of her anus, making it really painful to sit on a bike or the floor. We've tried everything is surgery the only solution left?

Had piles laser surgery 2 months back but now having trouble with bowel movements feels like anal stenosis since I strain a lot and need multiple bathroom trips to fully empty. Sometimes there's slight bleeding too. What could be going on?
