My current urea level is 47 and creatinine is 2.2. I have already undergone dialysis 6 times. What measures or treatments can help bring these levels back to normal?

A creatinine level of 2.2 and urea of 47 shows moderate kidney stress. After completing dialysis, its very important to protect kidney function through a low salt, low protein diet, controlled blood pressure/sugar, and regular lab monitoring. My recent blood test report shows: Creatinine: 1.39 mg/dL Uric Acid: 6.8 mg/dL Hemoglobin: 5.2 gm RBC: 5.1 million/cmm I am hypertensive and take Metoprolol 25 mg once daily and Telmisartan 20 mg twice daily.
