My father is 65 years old and is suffering from hernia. The doctor has advised surgery. During the pre operative examination, his creatinine level was found to be 2.3, and we were advised to consult a nephrologist for further evaluation. Does this creatinine level indicate kidney failure? If yes, to what extent, and how can it be managed? Also, can we safely proceed with the hernia operation? For context, he has diabetes, but his blood pressure and sugar levels are under control.

A creatinine level of 2.3 is higher than normal and may suggest moderate kidney function decline. It does not mean kidney failure, but it must be evaluated before surgery. Since your father has diabetes, regular kidney checks are important. A nephrologist can guide if its safe to proceed with surgery and how to protect kidney function.

Recently, he had a problem where he was not able to urinate properly, but there was no pain. Creatinine level is 0.88 to 1.2, checked in different labs for repeatability. After consulting two different nephrology doctors, we are confused. One suggests normal kidney function, and the other suggests 20 percent kidney damage with a recommendation for Renosave medicine. We would like to know whether we need to take any medicine to prevent further kidney damage or if we should monitor eGFR to see any abnormality by creatinine level in blood tests? As per doctors suggestion, a DTPA test was conducted to examine kidney function. Following are the test report comments: There is abnormally increased circulating background tracer noted, suggestive of renal parenchymal dysfunction. Bilateral subnormally functioning kidneys without obstruction. Low normal limit of GFR for age greater than 70 ml/min. Kindly clarify and also suggest whether we need to take preventive medicine in this case or not to avoid any kidney damage?
