My mother is around 70 years old. She is suffering from joint pain. Nowadays swelling is observed in her foot. She also has diabetes and high blood pressure, for which she is taking medication. Low hemoglobin and very high ESR suggest chronic inflammation or arthritis. Consult a rheumatologist and physician. Anaemia should also be treated. Do the required iron studies and arthritis panel.
