My mom needs cataract surgery and the doctor gave two options a 2.8mm incision with an Indian lens or a 1.8mm one with an imported lens. Which one is better for her? I'm a bit confused about the difference in incision size and lens quality

Any lens can be implemented ie Indian or imported through either incisions. And results will be almost equal. So please look into your budget.
